Announcement: Horizon IEHP Transition to PHN
Starting June 1st, 2025, IEHP patients who were previously assigned to the Horizon IPA will be transitioned to a new network called Physicians Health Network, or PHN.
There’s no action needed on your part, and your coverage and benefits will stay the same. IEHP has already made the update in their system, so there shouldn’t be any disruption to your care.
If you’re currently seeing a specialist, your existing referral is still valid for 90 days. Any new referrals will now be processed through PHN. And if your specialist isn’t currently part of PHN, just let us know — we’ll work with PHN to help get them contracted.
